Honor Community Health’s Remote Care Team

Honor's team provides medical services to those experiencing homelessness in Oakland County. They aim to improve individual’s health status, help them establish ongoing medical care, and connect them to appropriate resources.

Services they Provide:
-Diagnose and treat medical conditions
-Prescribe medications
-Order labs and imaging
-Connection to behavioral health services
-Connection to substance use treatment services
-Health education
-Care coordination
-Insurance enrollment assistance

Their Current Schedule:

• Monday AM: Royal Oak First United Methodist, 320 W 7th St Royal Oak MI (anyone seeking medical services can be seen)
   

• Monday PM: Hope Recuperative Shelter, 1416 Joslyn Rd Pontiac MI (only those staying at this shelter can be seen)
   

• Wednesday AM: Wellness Wednesday, 212 Baldwin Ave Pontiac MI (anyone seeking medical services can be seen)
   

• Wednesday PM: Pontiac Pubic Library, 60 E Pike St Pontiac MI (anyone seeking medical services can be seen)
   

• Thursday all day: Hope Shelter, 249 Baldwin Ave Pontiac MI (only those staying at this shelter can be seen)
